Is it a Variceal Bleed?
Known Varices
Known Cirrhosis
Jaundice
Ascites
Spider Nevi
Splenomegaly
Platelets <100
If YES, give:
Terlipressin 2mg IV Stat
Co-amoxiclav 1.2g
INR correction if INR >1.5 (D/w haematology or ED senior)
In children always consider… Button Battery Ingestion!
Why?
Varices are the cause of 5-12% of AUGIBs
Mortality for all bleeds is 10-14% (varices increase the risk of mortality)
Early endoscopy (in appropriate patients) provides diagnosis, prognosis and allows immediate therapy
