CC11: Managing Long-term Conditions & Promoting Patient Self-care
Recall the natural history of diseases that run a chronic course. Define the role of rehabilitation services and the multidisciplinary team to facilitate long-term care. Outline the concept of quality of life and how this can be measured. Promote and encourage involvement of patients in appropriate support networks, both to receive support and to give support to others.
